What a pet vaccination record is

A vaccination record is proof that a pet's core and lifestyle shots are current. Boarding facilities, daycares, groomers and travel authorities routinely ask for it before accepting a pet. It lists each vaccine, the date it was given, when it's due again, and the clinic that administered it.

This template lets you draft and print a clean copy. It does not replace your veterinarian's official certificate — always confirm every date against the clinic's records. Pet-care businesses that collect these at intake usually store them in pet-care business software, which flags expiring vaccines before a stay and keeps a copy on the client's file.

Vaccination Record FAQ

Most facilities require rabies, DHPP (distemper/parvo) and Bordetella at a minimum, and many ask for canine influenza or leptospirosis too. Requirements vary by facility and state, so check with each business and your veterinarian.

No. This is a free template to help you compile and print a record. The authoritative document is the certificate your veterinarian issues. Use this to organize the information, then confirm dates against the clinic's paperwork.

The cleanest approach is digital intake: clients upload the record once, and the software tracks each due date and blocks bookings when a required vaccine has lapsed. That removes the front-desk scramble of checking paper certificates at drop-off.
